LLR PCL makes a significant positive difference in primary care provision. Our aim is to facilitate efficient and effective, high quality healthcare services which benefit both patients and our member practices. We are committed to developing and improving our offering by continuously seeking and responding to feedback from both patients and clinicians.
Here are some of the reasons why you should consider joining us.
Primary Care Resilience
We believe there is a wealth of skill and experience available amongst our community’s primary healthcare providers, which can be of benefit to all. We want to support primary care resilience by encouraging your practice to maximise the services (above core contract) you are able to offer patients and be paid for this. We are committed to identifying the right care for patients and matching it with the right service, provided within the community by an LLR PCL provider practices.
Complete Information Integration
All of our provider practices are provided with a PCL SystmOne Community Hub and so are visible to the Referral Support Service. This means that the RSS Triager is able to identify where a patient can receive specialist treatment within primary care, at a PCL provider practice. This system is not only efficient for both the patient and the NHS, it also generates reports for payment.
Total Investment in Primary Care
As a Community Interest Company, we are run on a not-for-profit basis. This is the very cornerstone of what we do and means that all profits are re-invested for the betterment of community primary care. We are constantly looking for ways in which we can help support our shareholding practices in providing better care for patients.
As an example, we have launched a £400,000 equipment fund. Our shareholding practices will be able to access this fund, leasing equipment at a highly competitive, less-than-market rate over a pre-agreed timescale. At the end of the lease, the practice will then be able to purchase the equipment outright for a nominal sum. This means our practices no longer have the burden of taking out expensive bank loans in order to develop their services.
Stronger Together
LLR PCL funds and facilitates the PCL Partnership Board which is an essential forum for improving communication, conditions and care within Primary Care Networks. The 26 PCN’s in our area are represented by 9 delegates who sit on the board. By co-ordinating this forum and corralling opinion, we are able to strengthen the voice of primary care. LLR PCL now represents a significant number of shareholding practices in our region. This means that we have a stronger presence within the Integrated Care Board.
Support For Development
We strongly believe in making the most of what you have to offer. If you would like to become a provider practice but are concerned you do not have the necessary qualifications or skills to offer a service, we are still keen to talk to you. We are linked to the LLR Training Hub (run by Health Education England) and can support you by organising training for your team. Our equipment fund can also help you if you require additional kit to bring a service online.
Closing The Care Gap
We are committed to supporting our NHS by looking at ways to relieve pressure in primary and secondary care. We identify opportunities in primary care which could be appropriately funded and developed so that patients can remain in their community, preventing them having to enter secondary care. Similarly we want to see patients moved out of secondary care as quickly as possible, receiving appropriate care in their local environment. LLR PCL supports the provision of better care, quicker and closer to home. We also want to ensure that our provider practices are appropriately rewarded for the care and support they offer patients. Our aim is to enhance opportunities for primary care and ultimately provide more care closer to home for patients.
Cross-Community Relationships
LLR PCL has developed deep community relationships which reach across all levels. We are able to bring people together to better serve our community. We work with charities and volunteering organisations, faith groups, local councils, diagnostic organisations as well as NHS bodies. We are able to co-ordinate and facilitate projects which serve the health and well-being of the people of Leicester, Leicestershire and Rutland. This was never better illustrated than by the Covid-19 Vaccination program, where LLR PCL played a key facilitative role.
Enhanced Access
PCL formed a collaboration with Derbyshire Health United CIC and Phoenix Healthcare Partnership to provide an Enhanced Access service for Melton, Syston and Vale, Rutland and Salutem Primary Care Networks. The service went live on 1st October 2022 and will run until March 2024. GPs provide remote telephone appointments on weekdays during the hours 18.30 – 20.00. Face-to-Face nursing appointments are provided on a Saturday, between 09.00 – 17.00.
Of patients were “Extremely satisfied” with their out of hours appointment.
Of patients rated the service they received at the reception areas as “Excellent” .
Of patients said their provider listened to their needs “Extremely well” .
Of patients said they were “Extremely satisfied” with the cleanliness and appearance of the facility.
Of patients were seen “on time” or “earlier” for their appointment.
MSK Hub
The LLR MSK Hub was launched in February 2022. The Hub now hosts three specialist clinicians across three sites, covering Leicester City, East and West. Since launch the programme has seen over 1,300 patients across all three sites. Very few of these patients required onward referral.
This service was one the first in LLR to enable two-way ICE Radiology communication via SystmOne Community Service units, ensuring the utmost safety for patients.

Breast Pain
Since the Breast Pain service has launched, the programme has seen over 680 women. The service is provided over three locations in LLR. The service is to be reviewed by independent auditors, Edge Health. This audit is to assess the impact on the 2 Week Wait department, outcomes of referral and treatment and the overall patient care journey. It is hoped that the programmes effectiveness and cost efficiency will result in the service continuing.

Case Study - Cardiorespiratory Diagnostics Service
PCL launched a community-based Cardiorespiratory Diagnostic Service in September 2023 which will run until March 2024. Historically, these kind of diagnostic services have never been commissioned on a primary care footprint. Thus, certainly within the Midlands, this service is unique in its model of local provision. We are hoping for on-going funding.

setting has led to an uneven delivery of vital investigations. Practices that wish to perform 12 lead ECG, for example, have had to but
thenecessary hardware themselves and perform the investigation unfunded. (Interpretation is then necsessary.). This has contributed to the huge inequalities of health that we see across the region. There is truly a postcode lottery of diagnosis of atrial fibrillation, hypertension and chronic obstructive airways disease, amongst others.
With the commissioning of ECG, 24 Hour Holter and BP, Spirometry and
FENO within primary care, LLR are at the forefront of modernising this neglected area.”

Case Study - Roving Healthcare Units
The Beaumont Leys Health Centre recently used the Roving Healthcare Units to promote and deploy child immunisation as well as a number of other services, to great success.
“The Roving Healthcare Units service has been so valuable. We have managed to reach the heart of our community and offer child immunisation - MMR, Pneumonia and Tetanus being available. We also carried out Asthma checks, Blood pressure and COPD checks. The feedback from the patients has been amazing. Patients stated it was convenient and easy as no appointment needed to be arranged. Unfortunately we have a low engagement at this surgery. However this was not reflected in the turn-out for the Roving Healthcare Unit. This all helps us with our Quality & Outcomes Framework points : patients turned up on the day as no appointments were necessary. SMS were sent to target patients and during the day there were a couple of occassions when there was actually a queue of patients waiting. They could turn up at any time and it was so convenient for them. We have previously tried walk-in events at our surgery however the Roving Healthcare Units proved to be far more successful. I would recommend this service as it’s a different way of reaching the community and promoting healthcare.
