Patient Safety Incident Response Plan

On the very rare occasion that something goes wrong with medical treatment and care, it is vital that there is a clear set of guidelines in place, which clinicians can follow to ensure that the incident is dealt with as efficiently, thoroughly and compassionately as possible.

The Patient Safety Incident Response Framework has been developed by the NHS to set out clear and effective systems and procedures for responding to incidents involving patient safety.  The framework ensures consistent actions are undertaken when dealing with such circumstances.  It also allows for the opportunity to learn and improve patient safety by increasing understanding of the factors which contributed towards the incident taking place.

The PSIRF ensures a response plan is in place and integrates these four key aims:

  • Compassionate engagement and involvement of those affected by patient safety incidents.
  • Application of a range of system-based approaches to learns from patient safety incidents.
  • Application of a range of system-based approaches to learns from patient safety incidents. Considered and proportionate responses to patient safety incidents.
  • Supportive oversight focused on strengthening response system functioning and improvement.
